Qualifying for a Service Dog

The process of applying for a service dog requires:

  1. A completed application,
  2. A recent photo of the applicant,
  3. A formal release (enabling D.U.O. Assistance Dogs, Inc. to store, process, and share HIPAA-protected information with authorized personnel),
  4. A physician's attestation,
  5. and A veterinary reference form.

A $25 non-refundable application fee is required.

Compliance with the application process is essential. At this time D.U.O. does not provide assistance dogs to individuals with total hearing loss, or to those requiring a guide dog for visual impairment, or to serve psychiatric needs including but not limited to bipolar disorder, Dissociative Identity Disorder, or schizophrenia.
